Renal Tubular Acidosis , Distal Type with Rickets : A Case Report

Abstract

|
Rickets or osteomalacia occurs in patients with renal tubular acidosis, a. syndrome in wfr, metabolic acidosis results from abnormalities of renal"ihbular function. Type I ("distal", "classic" RTA) is ¢¥associated with a defect in the mechanism by which the distal tubule generates and maintains a hydrogen ion gradient and Type 1( ("proximal", "bicarbonate wasting" RTA) is associated with a defect in renal tubular reabsorption of filtrered bicarbonate. Recently authors experienced a 16-yr-old male patient who was admitted to the Seoul National University Hospital due to growth retardation and waddling gait. Urine was alkaline despite NH4C1 loading and metabolic acidosis was rather mild as shown by arterial blood gas analysis. X-ray studies and biopsy from iliac bone showed findings compatible with osteomalacia and nephrocalcinosis was also demonstrated.radiologically and pathologically, but glomerular function was intact. Other findings were high serum alkaline phosphatase, low phosphorus and hypokalemia.. He was managed with bicarbonate and, phosphate and moderate improvement of symptoms appered 3months after treatment.
